Patient Information

Please bring a copy of your current insurance cards.  It is also helpful if you bring any copies of old or current spectacle or contact lens prescriptions. If you are a contact lens wearer it is helpful to bring your old box or current boxes and contact lenses if you do not have a CL prescription.  We look forward to meeting you.

Make Payment Online

Now you can make a secure payment on your account with Johnston Eye Associates. If you have an account balance with Physicians Optical, this needs to be paid separately. Please call Physicians Optical at 405 749-4285 during normal business hours.

Frequently Asked Questions

What Medical Insurances do we accept?

We accept most of the big major medical insurance plans in Oklahoma, including Medicare, Blue Cross  Blue Shield,  United Healthcare, Humana, Aetna, Cigna, Tricare, and Medica.  We accept a large variety of medical insurance plans. If you are on a plan not listed above please call our office prior to your visit to verify coverage.  1-405-749-4280.

What Vison Insurances do we accept?

We accept VSP ( Vision Service Plan). We can also file Eyemed claims for you, even though we are an out of network provider.

Will I have to pay any out of pocket costs at the time of my exam?

At the time of your exam, you will be responsible for any insurance co-payments. we are seen as a specialist provider by most insurance companies, not a primary care provider. This may effect your co-payment amount due. You may also be responsible for the refraction fee ( spectacle power check) as this is most often a non-covered service.  Contact lens fitting fees are also non covered by medical insurance.   Vision Insurances have co- payments as well . Please check your insurance prior to your visit to be familiar with the amount of these co- payments.

Do we accept Medicaid?

At this time, we do not accept  Medicaid.

Can I pay cash for my exam?

Yes, we can accept cash at the time of your exam.

What is the general wait time for an exam?

We can usually see most patients within a week. We do keep some exam appointments open only for new patients on most days, so often we can see you sooner then a week.
